Role description
We are looking for a Senior FHIR Data Engineer to lead the migration of a legacy Azure API for FHIR implementation to Azure Health Data Services (AHDS). This role is deeply technical and specialized β ideal for engineers who have worked directly with FHIR R4, SMART on FHIR, and CMS interoperability APIs.
As the technical lead, you will architect, secure, and optimize real-time healthcare data pipelines, enabling scalable and compliant patient data exchange in line with CMS and HIPAA requirements.
What Youβll Do
Migrate existing Azure API for FHIR workloads to Azure Health Data Services (AHDS)
Implement SMART on FHIR (Enhanced) for secure and scalable health data access
Develop and secure FHIR-based APIs (Patient Access, Provider Directory, Prior Auth) using OAuth 2.0, RBAC, private endpoints, and WAF
Build FHIR-conformant resource parsers, bulk and delta data loaders, and transformation services
Create infrastructure using IaC (Bicep or ARM templates) and automate monitoring with Azure Monitor & App Insights
Work across DevOps, Security, and Architecture teams to ensure regulatory compliance and robust deployment
Lead release planning, UAT coordination, performance testing, and production rollout
Deliver key assets: OpenAPI specs, SOPs, RBAC policies, and pipeline documentation
Must-Have Experience
7+ years in cloud or healthcare data engineering
3+ years building or migrating workloads on Azure API for FHIR or AHDS
Demonstrated experience with:
FHIR R4 and SMART on FHIR protocols
CMS Patient Access and Payer APIs
HL7v2 message processing (ADT/ORM/ORU)
Azure ecosystem tools: APIM, Data Factory, Synapse, AD, Monitor
IaC with Bicep, ARM, Terraform, or similar
API security: OAuth 2.0, JWT, WAF, RBAC, NSGs
CI/CD with GitHub Actions or Azure DevOps
Nice-to-Have Experience
Exposure to Da Vinci PDex, Prior Authorization IGs
Experience tuning and scaling FHIR server workloads in production
Familiarity with CMS Interoperability Rules, ONC compliance frameworks
Azure certifications: AZ-305, FHIR Proficiency, Healthcare AI/MLOps
