Job Summary
Direct Difference is seeking an experienced Sepsis Abstractor to perform high-quality clinical data abstraction of patient records. This role supports clinical process improvement, patient safety initiatives, and compliance with CMS Core Measures and Joint Commission standards. The Sepsis Abstractor will ensure accurate, complete, and timely abstraction of patient data.
This is a 100% remote, flexible position where you can abstract on your own schedule. A personal device is required. Compensation ranges from $45–$55 per hour, depending on experience and performance.
Key Responsibilities
Data Collection & Abstraction
• Abstracts clinical data from patient medical records for Sepsis Core Measures.
• Ensures accurate, complete, and timely data entry into vendor platforms.
• Maintains an abstraction accuracy threshold of ≥95%.
• Adheres to CMS and The Joint Commission specification guidelines.
Quality & Reporting
• Participates in quality assurance activities, including data validation and audits.
• Communicates data documentation needs to support reliable reporting.
• Identifies and recommends improvements in abstraction and documentation processes.
Collaboration & Support
• Works under facility Account Managers that report to unit-based staff, performance improvement teams, and physician quality task forces.
• Serves as a subject-matter resource on sepsis abstraction for internal and external stakeholders.
• Attends program teleconferences and professional development sessions as required (e.g., CMS Sepsis updates).
Qualifications
Required
• Minimum 2 years’ recent experience in Sepsis abstraction.
• Strong knowledge of multiple EMR’s Vendor tools and CMS Sepsis specifications manual.
• Proficiency with Microsoft Office, databases, and abstraction software.
• Ability to work independently and maintain strict confidentiality.
• Exceptional attention to detail and organizational skills.
• Personal device required for remote abstraction.
Preferred
• Licensed LPN or RN (State Board of Nursing).
• BSN preferred.
• CPHQ certification a plus.
• CMS IQR Validation experience.
• Experience with remote access systems (e.g., Citrix, VMware, AnyConnect).
• Flexibility in scheduling and ability to manage multiple accounts concurrently.
Note: Education/licensure may be waived with 3+ years of abstraction experience and proven accuracy >95% with external validation.
